To order a repeat prescription or a further social welfare cert, please complete and submit this form. This form is to be used only if you are a registered patient of Newtown & Monksland Medical Centres and are ordering medications we have prescribed in the past, and/or require ongoing medical certification (social welfare certs) for an existing issue.
New patients, or patients requesting new medications/new certification or with a new illness/injury, should call us on:
*Online and phone orders for repeat prescription and welfare cert requests will be dealt with between the hours of 11-12:30pm and 2-4:30pm daily. Please note we cannot facilitate orders outside of these hours.
Fees
Repeat prescriptions: A fee of €25 applies to repeat prescription orders for private patients. Once your order has been reviewed and accepted, a text link will be sent to your mobile phone to facilitate payment. Payment must be made prior to the issue of your prescription.
All repeat prescriptions will be issued to the pharmacy of your choice within 3 working days (excluding weekends and Bank Holidays). The period of 3 working days commences when your order has been accepted.
Social welfare certs: There is a fee of €20 for each certificate issued to our private patients. A link will be texted to you to pay online and your cert will be forwarded to your chosen address when payment has been received.
